直肠周围间隙
直肠周围筋膜及间隙的背外侧解剖。
A,手术中分离Toldt间隙,向下拓展进入直肠后即为直肠后间隙(腹侧为直肠系膜筋膜,背侧为泌尿生殖层脏层),
B,解剖泌尿生殖层进入底前间隙(腹侧为泌尿生殖层壁层,背侧为骶前间隙)。泌尿生殖层包裹着腹下神经和输尿管。
C,骶前筋膜可以进一步从骶骨剥离,进入肛提肌上间隙(腹侧为骶前筋膜,背侧为骶骨骨膜)。
最后一图为矢状面示意图,可见泌尿生殖层在直肠背侧的附着处。
